The Support at Home Service launched in 2012 incorporates a range of services, most of which are short term, and include re-ablement, crisis care and home safety services (technology). The purpose of each of the services is to provide a range of supports , whether through the use of technology(telecare and tele-health equipment), by means of assessment and supply of appropriate equipment, or by providing a team of re-ablement staff to assess, agree a care plan and provide staff to support and encourage service users to carry out their daily living activities in the short term , or through time limited care supports from crisis care. All of these activities are aimed at encouraging service users to be as independent as they can be.

The Support at Home Service is now looking to recruit a full time team leader, who will work within a multi-disciplinary team based in a hub at St John's hospital . The post-holder will be part of a multi-disciplinary team, which will include consultants, nursing staff, occupational therapists and physiotherapists. The function of the hub is to be able to facilitate a return home for patients who will be referred to the hub. The job scope is as follows
  • Be able to work on their own initiative and take decisions within the policy framework, standards and objectives of the service
  • Offer advice and support to health colleagues in relation to referrals for re-ablement, home safety and crisis care services
  • Offer advice, support and be liaison between health colleagues, social work practice teams and framework care providers

Applicants must have
  • Good communication skills
  • Good literacy and numeracy skills
  • Comfortable/confident with a range of IT systems
  • Working knowledge of social work practice teams and referral systems
  • Understanding of role of framework care providers

Successful candidates (for registered posts),if not already in possession of an SVQ Level III Health and Social Care or equivalent along with PDA in Supervision would be expected to make a commitment to achieving these qualifications within 6 months of taking up post.

This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
