Our community based service engages with LD adults in a variety of settings – day centres, supportive living accommodation and individual homes. Interventions are implemented in the most appropriate and effective manner to maintain the functional skills and health of our clients.

If you have a holistic approach to care provision and experience of community multidisciplinary working then we would love to hear from you. This post is being offered as 0.8 wte – 30 hours - if you are interested but would prefer alternative hours we would still like to hear from you.
